Element: Obtain and record consent from a member and their family before referral to a support service in accordance with organisational procedures
       
Element: Make referrals that address the needs of the member and their family according to organisational policies and procedures
       
Element: Obtain feedback from the member and their family on how the PND process assisted them

Assessment Tasks
